Here’s how the service works:
1. Complete the Travel Health Questionnaire: Start by completing our online form to share your travel plans and vaccination history.
2. Consultation and Review: An independent prescriber will review your form. If appropriate, a prescription will be issued and sent directly to the pharmacy.
3. Schedule Your Appointment: Please contact D H Roberts Pharmacy two days after submitting your form to book your vaccination appointment.
